You can unlock new locked difficulties by achieving an A grade on the prior difficulty!

Example:

• Beating ‘Intermediate’ with an A unlocks ‘Hard’

For the time being I’m not planning on adding any more difficulties as Insane is hard enough as is. If you have any cool ideas though, be sure to shoot me a message and I might make it part of the game!
